Reconstitute to 4 mg/mL for IV administration.

IV administration

Give reconstituted solution over at least 2 minutes or further dilute to 0.4 to 0.8 mg/mL and give over 15 minutes.

May dilute to 0.8 mg/mL or weaker and give as a continuous infusion.

RCH Standard Concentrations: 0.8 mg/mL

Other routes of administration

IM: Not recommended.

SC: Not recommended.

Pantoprazole 0.8 mg/mL or weaker:

Glucose 5%

Hartmann’s

Pantoprazole 0.4 mg/mL or weaker:

Glucose 10%

Y-site only and pantoprazole (Sandoz and Sun brands only) 0.8 mg/mL or weaker:

Potassium chloride 20 mmol/L

Additional information

Reconstituted vial is stable for 12 hours when refrigerated.

Protect from light during storage.
