IV administration
Dilute to 12 mg/mL or weaker and infuse via a large peripheral vein with adequate blood flow or a central line only.
May give undiluted via a central line only.
Doses equal to or less than 60 mg/kg: infuse over at least 1 hour.
Doses greater than 60 mg/kg: infuse over at least 2 hours.
Maximum rate: 1 mg/kg/minute.
Other routes of administration
IM: Not recommended.
SC: Not recommended.

Additional information
Extravasation risk: vascular irritant.
Potentially hazardous medication: may require special handling precautions; refer to RCH Safe Handling of Hazardous Medicines Procedure or relevant local guideline.
Ensure adequate hydration to reduce renal toxicity.
Use with caution in patients predisposed to QT interval prolongation or in patients being administered other medications known to prolong the QT interval.
Monitor electrolytes closely as imbalance may precipitate seizures; use cautiously in patients with seizure history.
Discard any unused vial contents.
