Reconstitute to 50 mg/mL for IV administration. Swirl gently to dissolve.

IV administration

Dilute to 0.5 to 5 mg/mL and give as per local protocol.

RCH Standard Concentrations:

0 to < 5 kg: 25 mg/50 mL (0.5 mg/mL)

5 to < 10 kg: 50 mg/50 mL (1 mg/mL)

10 to < 20 kg: 125 mg/50 mL (2.5 mg/mL)

20 kg+:  250 mg/50 mL (5 mg/mL)

Other routes of administration

IM: Not recommended.

SC: Not recommended.

Compatible IV fluids

Glucose 5%

Sodium chloride 0.9%

Y-site only and bivalirudin in glucose 5%:

Potassium chloride 20 mmol/L

Additional information

Reconstituted solution should be clear to opalescent, colourless to slightly yellow.
