Reconstitute to 5 mg/mL and shake until solution is clear.
IV administration
Do not administer without further dilution.
Flush IV line with glucose 5% prior to and after administration.
Dilute to 0.1 mg/mL or weaker and give over 6 hours. Infuse at a controlled rate using a syringe driver or infusion pump.
May be given as a continuous infusion if nephrotoxicity is suspected or observed. Slower infusion rates may reduce the efficacy by lowering the maximum concentration.
Minimum infusion time is 2 hours. Faster infusion rates are associated with an increased risk of nephrotoxicity and infusion reactions.
Maximum concentration if necessary: 0.4 mg/mL via a central line only.
An in-line filter of at least 1 micron may be used.
RCH Standard Concentrations:

Additional information
Extravasation risk.
Observe closely for anaphylactic reactions.
Use only when facilities for cardiac monitoring and cardio-respiratory resuscitation exist.
Premedication with corticosteroids, antipyretic agents and/or antihistamine prior to the infusion is recommended and may reduce the incidence and intensity of side effects during the infusion. Refer to relevant local guideline.
A test dose may be recommended; refer to local guidelines.
Monitor patient for infusion reactions and reduce rate of administration if not tolerated.
Ensure adequate hydration.
Refrigerate, do not freeze.
Reconstituted solution should be clear and yellow.
Protect from light during storage.
Protect reconstituted vial from light during storage.
