Amikacin (DBL), Amikacin (SXP), Amikacin (Wockhardt)
250 mg/mL (volume: 2 mL)
Dilute to 5 mg/mL or weaker and infuse over at least 30 minutes.
Infuse over 1 – 2 hours in neonates and infants.
Maximum concentration if necessary: 10 mg/mL.
Other routes of administration
IM: Suitable. Give undiluted.
SC: Not recommended.

Sodium chloride 0.9%
Amikacin 5 mg/mL or weaker:
Glucose 5% and sodium chloride 0.45%
Glucose 5% and sodium chloride 0.9%
Potassium chloride 20 mmol/L
Therapeutic Drug Monitoring required; refer to local guidelines.
IV aminoglycoside antibiotics are inactivated by IV cephalosporins, penicillins and teicoplanin; refer to Glossary and Notes.
Discard any unused vial contents.
