Presentation and preparation
250 mg/mL (volume: 2 mL)
IV administration
Dilute to 5 mg/mL or weaker and infuse over 30 minutes to 1 hour.
Neonates and infants: Dilute to 5 mg/mL or weaker and infuse over 1 to 2 hours.
Maximum concentration if necessary: 10 mg/mL.
Haemodialysis: Give at the end of haemodialysis.
Other routes of administration
IM: Suitable. Give undiluted.
SC: Not recommended.

IV aminoglycoside antibiotics are inactivated by IV cephalosporins, penicillins and teicoplanin; refer to Glossary and Notes.
Ensure adequate hydration.
Therapeutic Drug Monitoring required; refer to local guidelines.
Solution should be clear and colourless to pale-yellow.
Discard any unused vial contents.
